Description

American Traveler is hiring an experienced RN for a Home Health Field RN position in Bastrop, TX, requiring HCHB and OASIS experience with the ability to hit the ground running in a fast-paced home health setting.

Details

• Field-based home health setting covering a service territory up to 50 miles outside the home base
• Geriatric patient population with a focus on in-home skilled nursing care, interdisciplinary care coordination, and plan of care development
• Average caseload of 4–6 patients per day, with approximately 24–30 points per week (equivalent to 40 hours per week)
• Charting is performed using HomeCarHomeBase (HCHB) and OASIS, and must be completed while at the patient's home
• Monday–Friday, day shift (8:00 AM – 5:00 PM)
• Weekends required as needed; participation in the on-call rotation is required, including approximately one night of call per week (details discussed at interview)

Requirements

• Active TX RN license or a Compact RN license currently in hand (pending licensure not accepted)
• Current BLS certification required
• Minimum 1 year of home health experience required; minimum 2 years of overall clinical experience required
• Proficiency with HCHB and OASIS required; 1 year of experience with both strongly preferred
• Willingness to travel up to 50 miles outside the home base location
• 2 professional references from a supervisor within the past 2 years required for consideration
• Full 7-year work history with all gaps explained required for consideration

Additional Information

• Responsible for administering skilled nursing care to patients from diverse populations across a variety of home environments
• Coordinates care with patients, families, certifying providers, and all members of the interdisciplinary team
• Performs comprehensive patient assessments and collaborates with the certifying provider to develop individualized plans of care
• Candidates must be non-smokers, as the patient population includes those with COPD who are sensitive to cigarette smoke
• Mileage is reimbursed at the IRS standard rate
• Scrubs in any color are acceptable
• Local candidates within a 50-mile radius are accepted
