Description

American Traveler is seeking a Home Health Physical Therapist with at least 1 year of skilled home health experience for a 13-week travel contract serving West Portland, OR and surrounding areas.

Details

• Home health setting serving West Portland, OR and surrounding areas
• 1:1 patient ratio with up to 6 patient visits per day; 30 productivity points is standard
• Schedule is 5x8-hour day shifts; 4x10 and 3x12 schedules are not available
• No weekend or on-call requirements
• Travelers are expected to float between all facilities within the state; refusal to float may result in termination of assignment
• Charting is done via Epic Remote Client for Home Health (on laptop) and Rover (phone app); Epic experience required
• Candidates within a 50–75 mile radius of the facility are considered local and are accepted at the same rate

Requirements

• Minimum 1 year of skilled home health PT experience required
• Current BLS certification required
• Active PT license required; license required if applicable
• Epic EMR experience required
• Must have reliable personal transportation
• Primary COVID-19 vaccine series plus one booster required
• Minimum of 2 supervisory references covering 1 year of employment within the last 3 years in a comparable specialty or setting required for consideration
• Full SSN and DOB required for consideration

Additional Information

• Responsibilities include evaluation and treatment of home health patients across West Portland and surrounding areas
• The scheduling system groups patients geographically to minimize drive time; occasional daily drive radius of up to ~60 miles (approximately 60–90 minutes total) is possible
• Mileage is reimbursed at the IRS standard rate
• Business casual attire required; scrubs are allowed but no specific color required — no jeans
• Former permanent employees of this facility must wait at least 3 months before returning as a traveler and cannot be within 50 miles of the submitting facility
• RTO is minimal; a few days may be approved but significant RTO requests are discouraged
• Open to first-time travelers
• PTA candidates may be considered if necessary
