Description

American Traveler is seeking a Home Health Physical Therapist with a valid CA PT License and OASIS experience for a 13-week travel assignment.

Details

• Home health setting covering a territory in South Marin County, including Sausalito, Tiburon, Belvedere, and Mill Valley
• Day shift schedule, 5x8-hour shifts from 8:00 AM to 5:00 PM
• Caseload of approximately 5 visits/points per day
• Uses Epic EMR with OASIS documentation required

Requirements

• Current CA PT License required (license required if applicable)
• BLS certification required
• Valid driver's license and auto insurance required
• Epic EMR experience required
• OASIS documentation experience required
• Minimum 2 years of experience required
• Prior travel experience required
• Strong critical thinking, independent judgment, and problem-solving skills required
• Effective verbal and written communication and interpersonal skills required

Additional Information

• Responsibilities include patient assessment, treatment planning, and delivering therapy interventions across orthopedic, neurologic, and cardiopulmonary conditions in patients' homes
• Skills applied may include gait training, therapeutic exercise, wound/skin integrity assessment, and use of adaptive or assistive devices
• Collaborates with interdisciplinary care team members to develop and adjust treatment plans
• Provides patient and family education on home exercise programs and safety
• Maximum RTO of 7 days per contract, with only 1 holiday request approved per contract
• Local candidates accepted within a 50-mile radius at the local rate
• Returning staff/travelers must have a 1-year separation before being eligible to return as a traveler
