Description

American Traveler is hiring an Occupational Therapist with 1+ year of Home Health experience for a 13-week travel contract in New Lenox, IL.

Details

• Home Health setting serving patients in the New Lenox, IL area
• Day shift, 8 hours per day
• No on-call or weekend requirements, though Saturday and Sunday rotation may apply as applicable
• Caseload includes patients requiring ADL independence intervention and start of care evaluations
• Documentation completed in an electronic medical record (EMR)

Requirements

• Minimum 1 year of Home Health OT experience required
• Valid driver's license and current auto insurance required
• Active Illinois OT license required (license required if applicable)
• Current BLS certification (AHA) required
• COVID vaccination required — declinations will not be approved
• OASIS and start of care experience strongly preferred
• Ability to transfer and position patients required
• 2 supervisory references required for consideration
• Skills checklist (completed within the past year) required for consideration

Additional Information

• Assess, plan, treat, and implement individualized Plans of Care (POC) for all assigned patients
• Collaborate with physicians and provide supervisory support to OTAs and OT/OTA students
• Apply evidence-based interventions in accordance with AOTA standards of care and code of ethics
• Maintain timely and accurate documentation of all patient care activities in the EMR
• Local candidates will be reviewed; candidates located outside the service territory may not be considered
• No more than 5 days of RTO may be requested during the contract
