Description
American Traveler is seeking a Physical Therapist with home health experience for a 13-week travel contract in a home health setting, requiring Homecare Homebase (HCHB) proficiency.
Details
- Home health care setting serving patients in their homes across an assigned coverage area
- Patient population includes CHF, orthopedic, and pneumonia diagnoses
- Daily point expectation of 6.5 points, with responsibilities including Start of Care (SOC), Resumption of Care (ROC), recertifications, evaluations, and routine visits
- Charting is performed using Homecare Homebase (HCHB)
- Schedule is Monday–Friday, 8am–5pm with rotating weekend shifts (8am–5pm)
- Holiday rotation is required
- 8-hour shift days
Requirements
- Minimum 1 year of physical therapy experience required; new graduates are discouraged from applying
- Home health experience is required
- Proficiency with Homecare Homebase (HCHB) is required
- Active BLS certification (AHA) required; pending certifications will not be accepted
- Active state or compact (NLC) PT license required for the state of placement; pending licenses will not be accepted — license required if applicable
- Two professional references required: one supervisor from within the last 12 months and one peer or supervisor from within the last 3 years, including dates of employment and eligibility for rehire
- A copy of a valid driver's license is required for consideration
Additional Information
- Responsibilities include conducting SOC, ROC, recertification, evaluation, and routine home visits
- A clinical manager or educator is available 24/7 for field emergencies and 8am–5pm for general support
- Orientation includes approximately 2 weeks with a work coach, including supervised field visits; orientation totals 12 hours
- Either scrubs or business casual attire is acceptable
- A phone and tablet will be provided for use during the assignment
- Profiles are sent directly to the manager for a phone interview; no clinical team review involved
- Local candidates are accepted for this position
