Description
American Traveler is seeking a Physical Therapist for a Home Health position requiring 1 year of certified home care and case management experience.
Details
- Home health setting covering Chelsea, East Boston, Revere, and Winthrop
- 13-week assignment with an 08:00-16:30 shift schedule
- Weekend rotation occurs once every 6 weeks
- Requires travel to patient homes as assigned
- Uses Epic for charting
Requirements
- Minimum 1 year of Certified Home Health Care experience required
- Minimum 1 year of Case Management experience required
- Experience must be specifically within a certified home health agency; hospice, private care, or standalone case management roles do not qualify
- License required if applicable
- Flu vaccination required with no religious or medical exemptions accepted
- Ability to travel between patient care locations as needed
Additional Information
- Responsibilities include assessing patient conditions, implementing care plans, and providing education and support to patients and families
- Performs admission processes and case management duties per organizational policy and standards
- Requires thorough, timely electronic documentation and interdisciplinary communication
- Care delivered in accordance with community health practice standards
- Start date contingent on completion of credentialing clearance
