Description
American Traveler is seeking a Physical Therapist with 1+ year of home health experience for a 13-week Home Health assignment on the beautiful island of Kaua'i, Hawai'i.
Details
- Home health setting serving a primarily geriatric patient population (kupuna) in their homes on Kaua'i
- Day shift schedule, 5x8-hour shifts (8:00 AM - 4:30 PM)
- Productivity requirement of 25 units per week
- Patient population includes neurological, orthopedic, and cardiopulmonary conditions including post-stroke, post-op spinal surgery, total hip/knee replacement, COPD, CHF, Parkinson's disease, dementia, and more
- Occasional floating to cover Oahu, Maui, and/or the Big Island may be required; all float-related expenses are covered
- HomeCare HomeBase (HCHB) used for documentation; experience preferred
- OASIS documentation required; more than 3 months of OASIS experience required
Requirements
- Minimum 1 year of recent home health PT experience required
- More than 3 months of OASIS documentation experience required
- Current BLS certification (AHA) required
- Active HI state PT license required; license required if applicable
- Proficiency in geriatric, neurological, and orthopedic patient care
- Experience with adaptive and assistive devices, gait training, home exercise program development, and plan of care management
- Skills in patient/family education, safety and injury prevention, and environmental barrier identification
- Travel experience not required; first-time travelers are welcome
Additional Information
- Responsibilities include evaluation, plan of care development, OASIS assessments, reassessments, goal setting, and discharge planning
- Preferred skills include wound care, lymphedema management, Multiple Sclerosis, spinal cord injury, TBI, and neurodevelopmental techniques (NDT)
- Scrubs in any color and closed-toed shoes are required
- Please be prepared to provide total months of start-of-care/OASIS experience and frequency of Medicare patient starts of care per week for consideration
