Description

American Traveler is hiring an experienced RN for a Cardiovascular ICU position requiring 2+ years of CVICU experience and an active MD or Compact RN license.

Details

  • 10-week contract in a Cardiovascular Intensive Care Unit (CVICU)
  • Rotating night/mid shifts, 3x12 hours, 19:00-07:30 with possible rotation to 11a-11p based on unit needs
  • 36-hour work week
  • Every third weekend required with possible additional weekend coverage based on unit needs
  • Holiday coverage required as scheduled
  • Floating to MSICU required as needed
  • On-call requirement of 24-32 hours per rotating 6-week schedule
  • Average daily census of 3-5 patients with 6 beds on the unit
  • Patient ratio of 1:2
  • Common diagnoses include post-op cardiac surgery (CABG, MVR, AVR), cardiac surgery readmissions, structural heart procedures (TAVR, mitral clips), STEMI/NSTEMI, cardiogenic shock, and Inari procedures
  • Special unit focus on first 6-hours post-op management including inotropic medications, hemodynamic instability, active cardiac output management, coagulopathy, open chest, OHS/Impella, ECMO (perfusionist on-site), CRRT (dialysis on-site), and IABP
  • Epic charting system experience required

Requirements

  • Active MD or Compact RN license required
  • BLS, ACLS, and CPI certifications required
  • NIHSS, OCN, and TNCC certifications preferred
  • Minimum 2 years of CVICU experience required
  • First-time travelers accepted if they have CVICU experience
  • Previous charge nurse experience preferred
  • Teaching hospital and community hospital experience preferred
  • Immediate post-op open heart experience required
  • Skills required include IV therapy, arterial line management, central line care, blood product administration, and PICC line management
  • Respiratory skills required including ventilator management, BiPAP/CPAP management, ABG interpretation, and suctioning
  • Cardiovascular skills required including CABG care, IABP, TAVR, valve repair/replacement, and pre/post cardiac cath care
  • Renal skills required including CRRT/CVVHD and management of acute/chronic renal failure
  • Medication management skills required including titration of cardiac and vasoactive drips, continuous IV sedation, and emergency medications
  • Neurological skills required including CVA/acute stroke management
  • Cardiac monitoring skills required including dysrhythmia interpretation/management and 12-lead EKG acquisition

Additional Information

  • Provide critical care nursing to complex cardiovascular surgical and medical patients
  • Manage patients on ECMO, Impella, IABP, and CRRT under active physician direction
  • Collaborate with perfusionists and dialysis staff who are on-site for advanced interventions
  • Travelers are expected to float within scope with no restrictions
  • Must reside 50+ miles from the facility unless locals are specifically accepted for this role
  • Former staff must have been separated from the health system for at least 12 months before returning as a traveler
  • Returning travelers must have at least a 6-month break from the system after completing an assignment
  • One extension typically allowed before a required 6-month break from the facility/unit
  • One major holiday off during the winter season may be permitted depending on unit needs

Pros

The charge nurses and patient population is friendly.

Cons

They will short staff you every day. I got attitude multiple times when trying to take lunch 9 hours into my shift because ‘we don’t really do that here.’ Incredibly frustrating staffing levels, especially on the oncology floor.
