Description
American Traveler is seeking a licensed Physical Therapist with a minimum of 2 years of SNF/ICF experience for a 13-week long-term care assignment in Hawaii.
Details
- Work setting is a Skilled Nursing Facility (SNF) and/or Intermediate Care Facility (ICF) serving veterans
- Day shift position with a 5-day work week; departmental coverage spans 7 days, so schedule will likely include one weekend day
- Floating to other units or facilities may be required based on census and staffing needs
- EMR systems include Net Health Rehab Optima and Epic
Requirements
- Minimum 2 years of experience in a SNF/ICF setting preferred
- Active State of Hawaii PT license in good standing required by start of assignment; license required if applicable
- Current BLS certification required
Additional Information
- Responsibilities include performing evaluations, reassessments, treatments, and clinical services consistent with licensure and scope of practice
- Develops and implements individualized treatment plans in collaboration with physicians and the interdisciplinary team
- Educates residents, families, and caregivers regarding treatment plans and patient progress
- Completes accurate and timely documentation in accordance with facility, state, federal, and payer requirements
- May supervise therapy assistants as applicable to role
- Must be available to work Hawaii State and Federal holidays as needed based on facility operations
- Orientation provided for the rehabilitation department, EMR documentation, and facility workflows
- Candidates must arrive the Thursday prior to the assignment start date for a required employee health appointment on Friday
