Description
American Traveler is hiring a travel Occupational Therapist with at least 2 years of SNF/ICF experience for a 13-week assignment at a state veterans home in Hawaii.
Details
- Long-term care setting including a Skilled Nursing Facility (SNF) and/or Intermediate Care Facility (ICF)
- Day shift position with variable scheduling across 7-day departmental coverage; typically scheduled 5 days/week, likely including one weekend day
- Patient population consists of veterans requiring rehabilitation services
- EMR systems include Net Health Rehab Optima and Epic (orientation provided)
- Floating to other units or facilities may be required based on census and staffing needs
Requirements
- Active State of Hawaii OT license in good standing required by start of assignment; license required if applicable
- Minimum 2 years of experience in a SNF/ICF setting preferred
- Current BLS certification required
Additional Information
- Provide rehabilitation services including evaluations, reassessments, treatments, and other clinical services consistent with licensure and scope of practice
- Develop and implement individualized treatment plans in collaboration with physicians and the interdisciplinary team
- Educate residents, families, and caregivers on treatment plans and patient progress
- Complete accurate, timely, and compliant documentation including progress notes, plan of care changes, and discharges per facility, state, federal, and payer requirements
- Supervise therapy assistants where applicable to role
- Must be available to work Hawaii State and Federal Holidays as required by facility operations
- Orientation to the rehabilitation department, EMR systems, and facility workflows is provided
- Candidates must arrive the Thursday prior to the assignment start date to attend an employee health appointment on Friday
