Description

American Traveler is hiring a travel Occupational Therapist with at least 2 years of SNF/ICF experience for a 13-week assignment at a state veterans home in Hawaii.

Details

  • Long-term care setting including a Skilled Nursing Facility (SNF) and/or Intermediate Care Facility (ICF)
  • Day shift position with variable scheduling across 7-day departmental coverage; typically scheduled 5 days/week, likely including one weekend day
  • Patient population consists of veterans requiring rehabilitation services
  • EMR systems include Net Health Rehab Optima and Epic (orientation provided)
  • Floating to other units or facilities may be required based on census and staffing needs

Requirements

  • Active State of Hawaii OT license in good standing required by start of assignment; license required if applicable
  • Minimum 2 years of experience in a SNF/ICF setting preferred
  • Current BLS certification required

Additional Information

  • Provide rehabilitation services including evaluations, reassessments, treatments, and other clinical services consistent with licensure and scope of practice
  • Develop and implement individualized treatment plans in collaboration with physicians and the interdisciplinary team
  • Educate residents, families, and caregivers on treatment plans and patient progress
  • Complete accurate, timely, and compliant documentation including progress notes, plan of care changes, and discharges per facility, state, federal, and payer requirements
  • Supervise therapy assistants where applicable to role
  • Must be available to work Hawaii State and Federal Holidays as required by facility operations
  • Orientation to the rehabilitation department, EMR systems, and facility workflows is provided
  • Candidates must arrive the Thursday prior to the assignment start date to attend an employee health appointment on Friday
