Description

American Traveler is hiring a Home Health RN with required OASIS and home health experience for a Case Manager role requiring a VT or compact RN license.

Details

Home health and hospice care setting serving patients of all backgrounds
Serves as Case Manager for cases where this role is the lead discipline
Day shift schedule Monday through Friday
Weekend rotation required approximately every 4th week
Homecare Home Base experience preferred

Requirements

Active VT or compact RN license required
BLS certification required
Minimum of 1 year of experience required
Home health experience required
OASIS experience required
Experience as a home care traveler with OASIS proficiency preferred
COVID-19 vaccination required with no exemptions

Additional Information

Provide individualized, goal-oriented nursing care using the nursing process in accordance with physician orders
Coordinate care as Case Manager when serving as the lead discipline
