Description
American Traveler is looking for a Home Health Physical Therapist with a minimum of 1 year of Home Health experience and required HCHB/Oasis proficiency.
Details
- Home Health setting covering the Santa Cruz / Watsonville area
- Patient population consists of geriatric home health patients
- Average caseload of 5-7 patients per day
- Schedule is Monday-Friday, 8:30am-5pm, with a 30 minute lunch
- No on-call or weekend requirements
- Uses HCHB EMR system
- Coverage area is within a 50 mile radius from the hub/office
- Mileage reimbursed at IRS standard rate between patient homes
Requirements
- Minimum 1 year of recent Home Health experience required
- Previous Home Health or Hospice experience required
- Current BLS certification required
- CA PT license required in hand (license required if applicable)
- HCHB charting experience required
- Oasis experience required
- First-time travelers will not be considered for this position
Additional Information
- Local candidates accepted at the travel rate
- Main responsibilities include evaluation and treatment of geriatric patients in the home health setting
