Requirements

• Current, valactive, unrestricted Speech-Language Pathology (SLP) state licensure
• BLS certification (American Heart Association) that not not expire prior to start
• 1 year of relical experience required, preferably in a clinic setting
• Experience with EPIC EMR/charting system
• Provide date of birth and last 4 digits of SSN for consideration
• License required if applicable

Additional Information

• Conduct initial and ongoing patient assessments, including OASIS Assessments at appropriate time points
• Develop and implement individualized speech therapy plans of care
• Communicate with physicians before start of care and as needed throughout treatment
• Provide education and instruction to patients, families, and care team members
• Accurately document care and submit required paperwork in a timely manner to maintain patient medical records
• Returning employees must be separated from any Intermountain facility for at least one year before becoming eligible for a travel assignment
