Description
American Traveler seeks a Speech-Language Pathologist with home health experience and current state licensure to provide speech therapy services and assessments to patients in their homes.

Requirements
- Current, valactive, unrestricted Speech-Language Pathology (SLP) state licensure
- BLS certification (American Heart Association) that not not expire prior to start
- 1 year of relical experience required, preferably in a clinic setting
- Experience with EPIC EMR/charting system
- Provide date of birth and last 4 digits of SSN for consideration
- License required if applicable
Additional Information
- Conduct initial and ongoing patient assessments, including OASIS Assessments at appropriate time points
- Develop and implement individualized speech therapy plans of care
- Communicate with physicians before start of care and as needed throughout treatment
- Provide education and instruction to patients, families, and care team members
- Accurately document care and submit required paperwork in a timely manner to maintain patient medical records
- Returning employees must be separated from any Intermountain facility for at least one year before becoming eligible for a travel assignment
