Description

American Traveler is seeking a Home Care Physical Therapist with home health and OASIS experience to provide one-on-one therapy in patients' homes.

Details

Home health setting providing one-on-one physical therapy care in patients' homes
Schedule is Tuesday through Saturday, 8am-5pm
13-week assignment with 2 open positions
Guaranteed 36 hours per week
McKesson/Netsmart EMR used

Requirements

Active Vermont PT license required (license required if applicable)
Minimum 1 year of experience required
Home health experience required
OASIS experience required, as candidate will case manage
BLS certification required
McKesson/Netsmart EMR experience strongly preferred
Proficiency in standard therapy measurement techniques including ROM, strength, and balance testing required

Additional Information

Responsibilities include assessing patient conditions and developing individualized care plans to maximize mobility and independence
Collaborate with an interdisciplinary team to ensure patient needs are met
Utilize a variety of therapeutic modalities to optimize patient care
Candidates should have no requested time off during the assignment
