Description
American Traveler is seeking a Physical Therapist for a Home Health assignment, requiring home health experience and a Michigan PT license.
Details
- Home health setting covering Van Buren, Calhoun, and Kalamazoo counties
- Day shift, 5x8 hours, 8:00 AM - 5:00 PM
- Full-time, 40 hours per week
- Weekend rotation required every 4-6 weeks
- No call requirement
- No floating requirement
Requirements
- Michigan PT license required (license required if applicable)
- BLS certification (AHA) required
- Minimum 1 year of experience required
- Home health experience required
- Current, active driver's license in good standing required
- Must have daily availability of a car for business use with proof of insurance
- Supervisor-level references required
- Proof of flu vaccination required with submission, or exemption request within 48 hours of signing
Additional Information
- Evaluate, plan, and administer physical therapy treatment for patients with injuries or musculoskeletal, nerve, and joint conditions
- Conduct standardized assessments to establish functional baselines and track patient progress
- Develop individualized education and treatment plans for patients and families
- Participate in interdisciplinary team discussions and rounds
- Provide direct patient care through individual and/or group therapy sessions
- May design and fabricate splints or other assistive devices
- Document evaluations, treatment plans, and progress in the electronic health record per compliance timelines
- May cross-train and provide coverage in other Rehabilitation Services departments as needed
- Local candidates accepted at the same rate with no radius restrictions
- RTO limited to no more than 10 days
- Previous internal travelers may return after 6 months; previous permanent staff may return after 1 year
- Non-billable training modules completed during orientation and annually, self-reported
