Description
American Traveler is seeking a Physical Therapist for a Skilled Nursing facility, requiring 3+ years of experience and skilled nursing background.

- Provides physical therapy services to maximize residents' functional abilities and independence in activities of daily living
- Documents evaluative and therapeutic interactions in the Electronic Medical Record
Requirements
- Active PT license in applicable state of work required (License required if applicable)
- Minimum 3 years of experience required
- Skilled nursing facility experience required
- BLS certification required
- Bachelor's degree required; Master's or Doctorate preferred
- Must be able to lift up to 50 pounds, with a 2-person lift required above 50 pounds
- Ability to speak, read, write, and understand written instructions in English
Additional Information
- Evaluates residents per department protocols and establishes therapeutic plans of care including functional goals, frequency, and duration of services
- Collaborates with physicians and healthcare team on plan of care and discharge needs, attending pertinent team meetings
- Monitors and modifies treatment plans based on resident progress toward functional goals
- Recommends and educates residents on appropriate assistive devices
- Refers and collaborates with Physical Therapist Assistants to ensure quality resident care
- May supervise assistant-level staff in accordance with state mandates and professional standards
- Provides mentorship to new staff during orientation
- Maintains department cleanliness and follows infection control procedures
- Strong interpersonal, communication, and time management skills required
- Ability to work collaboratively in an interdisciplinary team
