Description
American Traveler is looking for a Home Health Physical Therapist with 1+ year of skilled home health experience to serve patients in West Portland, OR and surrounding areas.
Details
- Home health setting covering West Portland, OR and surrounding areas
- Day shift schedule, 5x8-hour shifts from 8:00 AM to 4:30 PM
- Facility cannot accommodate 4x10 or 3x12 schedules
- 1:1 patient ratio, visiting up to 6 patients per day with a standard of 30 productivity points
- No weekend or on-call requirements
- No floating between like units required
- Uses Epic Remote Client for home health documentation (on laptop) and Rover app for mobile charting
- OASIS documentation required
- May involve occasional daily travel radius of up to 60 miles (60-90 minute total drive time) to minimize windshield time between clients
Requirements
- OR state license required
- Minimum 1 year of skilled home health experience required
- BLS certification required
- OASIS experience required; candidates without OASIS experience should be noted
- Manual Therapy certification required
- Hand Therapy and Wound Care certifications preferred
- Epic charting experience preferred
- Must have reliable personal transportation for daily travel between patient visits
- Primary series COVID-19 vaccination plus one booster required
- License required if applicable
Additional Information
- Primary responsibilities include home health physical therapy evaluations, treatment, and OASIS documentation for patients across a range of orthopedic, neurologic, and cardiopulmonary conditions
- Facility scheduling system groups clients to help minimize drive time between visits
- Business casual attire required; scrubs are allowed but not mandatory and no specific color is required
- Free parking available; mileage reimbursed at the IRS standard rate
- Open to first-time travelers who have prior home health experience
- Local candidates accepted within a 50-75 mile radius at the same pay rate
- Former Providence permanent staff must wait at least 3 months before returning as a traveler and must live outside a 50-mile radius of the facility
- RTO requests should be confirmed directly with the hiring manager
