Description

American Traveler is seeking a Physical Therapist for a Home Health assignment requiring 1+ year of Home Health experience and an active PT license.

Details

• Home Health setting covering Palo Alto and neighboring cities including Atherton, Cupertino, East Palo Alto, Los Altos, Los Altos Hills, Menlo Park, Mountain View, Portola Valley, Santa Clara, Stanford, Woodside, and Sunnyvale
• 13-week assignment
• Day shift schedule from 8:00am to 5:30pm with flexibility based on patient availability
• One weekend day per month required

Requirements

• Active state PT license required (license required if applicable)
• Minimum 1 year of experience required, including Home Health experience
• BLS certification required

Additional Information

• Primary responsibilities include restoring patient function and minimizing disability following disease or injury through medically prescribed physical therapy programs
• May supervise Physical Therapy Assistants
• Conducts initial admission/evaluation visits and notifies the Clinical Team Manager of OASIS scores within established timelines
• Documents observations, clinical findings, problems, skilled interventions/treatments, goals, and discharge plans
