Description

American Traveler is seeking a Physical Therapist for a Home Health assignment requiring 1+ year of home health experience and PT licensure in VT and NH or a compact license

Details

• Home health setting covering more than 70 towns across Vermont and New Hampshire, including White River Junction VT, Lebanon NH, Bradford VT, and Windsor VT areas
• Full time, day shift schedule
• 16-week assignment
• Average patient visit length is 45 minutes
• Work independently while collaborating closely with an interdisciplinary patient-care team

Requirements

• BS in Physical Therapy required
• Must hold active PT license in VT and NH, or an unencumbered compact license - license required if applicable
• Minimum 1 year of experience as a PT in a home healthcare setting required
• BLS certification required
• Rehabilitation certifications such as ASHA, CHT, or ACP training may be required

• Evaluate patients' needs for physical therapy and develop individualized treatment plans
• Carry out treatment programs in accordance with agency policies and professional standards of care
• Communicate and coordinate with all members of the interdisciplinary health care team
