Description

American Traveler is hiring an RN Case Manager for a Home Health position covering VT and NH communities, requiring a VT or compact RN license and 2 years of home health experience.

Details

• Home health and hospice setting serving over 70 towns across Vermont and New Hampshire
• Locations include Bradford VT, Randolph VT, Springfield VT, White River Junction VT, and Lebanon NH areas
• Full time, day shift schedule
• 13-week assignment length
• Case management role coordinating care with an interdisciplinary team

Requirements

• Active RN license in Vermont and New Hampshire or unencumbered compact license
• Current BLS certification required
• Graduate of an accredited school of nursing
• Minimum 2 years of Home Health experience required

Additional Information

• Develop and periodically review patient care plans in collaboration with physicians, patients, and caregivers
• Coordinate patient care through interdisciplinary communication and case conferences
• Provide ongoing assessment of patient's clinical, functional, and emotional status
• Communicate significant patient changes to physicians and care team colleagues
• Document evaluations, treatment goals, and objectives
• Teach patients and caregivers self-care techniques and provide medication, diet, and health counseling as ordered
• Work independently while remaining part of a cohesive patient-care team
