Description

American Traveler is hiring a Home Health RN in Mesa County, CO offering flexible weekday scheduling and requiring an active CO or compact RN license.

Details

• In-person home health position providing skilled, patient-centered care in patients' homes throughout Mesa County, CO
• Full-time schedule with flexible day-to-day workflows, Monday through Friday
• Includes participation in an on-call rotation
• Manageable caseloads with one-on-one patient care
• EMR documentation required, HCHB experience preferred
• Collaborates with LPNs, therapists, caregivers, and other providers as part of an interdisciplinary team

Requirements

• Active RN license in Colorado or active NLC compact license required
• Current BLS certification required
• Valid driver's license, reliable transportation, and auto insurance required
• Home health experience preferred but not required
• Strong assessment and critical thinking skills
• Ability to work independently and manage a structured caseload
• Comfort with documentation and EMR systems, HCHB preferred

Additional Information

• Perform comprehensive patient assessments and develop individualized plans of care
• Provide skilled nursing care and appropriate interventions
• Monitor patient progress and report changes to the care team
• Educate patients and families on medications, treatments, and care plans
• Complete timely and accurate documentation in the EMR
• Work in a supportive, interdisciplinary team environment with clear communication and reasonable expectations
