Description
American Traveler is hiring a Home Health RN in Mesa County, CO offering flexible weekday scheduling and requiring an active CO or compact RN license.
Details
- In-person home health position providing skilled, patient-centered care in patients' homes throughout Mesa County, CO
- Full-time schedule with flexible day-to-day workflows, Monday through Friday
- Includes participation in an on-call rotation
- Manageable caseloads with one-on-one patient care
- EMR documentation required, HCHB experience preferred
- Collaborates with LPNs, therapists, caregivers, and other providers as part of an interdisciplinary team
Requirements
- Active RN license in Colorado or active NLC compact license required
- Current BLS certification required
- Valid driver's license, reliable transportation, and auto insurance required
- Home health experience preferred but not required
- Strong assessment and critical thinking skills
- Ability to work independently and manage a structured caseload
- Comfort with documentation and EMR systems, HCHB preferred
Additional Information
- Perform comprehensive patient assessments and develop individualized plans of care
- Provide skilled nursing care and appropriate interventions
- Monitor patient progress and report changes to the care team
- Educate patients and families on medications, treatments, and care plans
- Complete timely and accurate documentation in the EMR
- Work in a supportive, interdisciplinary team environment with clear communication and reasonable expectations
