Description

American Traveler is seeking a Physical Therapist for a home health position providing in-home skilled therapy services to patients in Grand Junction.

Details

  • Home health setting providing in-home physical therapy services
  • Monday through Friday schedule, 8 am to 5 pm
  • Guaranteed 40 hours per week
  • Serves patients in the Grand Junction community
  • One-on-one, patient-centered visits
  • Uses Homecare Homebase (HCHB) for documentation

Requirements

  • Current, unrestricted Colorado PT license required (license required if applicable)
  • Graduation from an accredited Physical Therapy program
  • Valid driver's license and reliable transportation required
  • Ability to practice independently in a home-based setting
  • Proficiency or willingness to learn HCHB
  • No minimum years of experience required
  • Home health experience preferred
  • Familiarity with Medicare documentation and home health regulations preferred
  • Experience with geriatric and medically complex populations preferred

Additional Information

  • Perform physical therapy visits including evaluations, Start of Care, reassessments, recertifications, and discharge visits
  • Develop and implement individualized plans of care based on patient needs and physician orders
  • Provide skilled interventions such as gait training, balance training, therapeutic exercise, mobility training, and fall prevention
  • Educate patients and caregivers on treatment plans, home exercise programs, and safety strategies
  • Communicate patient progress and condition changes to the interdisciplinary care team
  • May provide clinical oversight of Physical Therapist Assistants as applicable and permitted under Colorado law and agency policy
  • Collaborate with RNs, PTAs, OTs, STs, and agency leadership on care coordination
  • Ensure documentation complies with Medicare Conditions of Participation and HIPAA requirements
  • Mission-driven home health services with supportive clinical and administrative coordination
