Description
American Traveler is hiring a Home Health/Hospice RN requiring recent home health experience and OASIS documentation proficiency for a 13-week assignment.
Details
- Home Health and Hospice unit in a post-acute care setting
- Patient population includes CHF, diabetes, renal disease, wound care, IV therapy, and management of drains and other devices
- Typical caseload includes 6 points per day, such as an admission plus 3-4 patients
- Responsible for SOCs, re-certs, and resumptions
- Works alongside CNAs who provide home health/hospice aide support
- Schedule is Monday-Friday 0800-1630 with rotating weekends and holidays
- Weekend rotation typically occurs every 4th or 5th weekend with a day off before and after
- Holiday coverage required for up to two holidays per year
- On-call rotation includes one weekend per month and one evening per week
- May be required to see hospice patients while on call, with training provided
- Uses a computerized charting system for documentation
Requirements
- Active RN license in Michigan required
- Current BLS (AHA) certification required
- Minimum 1 year of nursing experience required
- Recent home health experience within the last 2 years required
- OASIS documentation proficiency required
- Wound care experience required
- Strong infection control practices in the home environment required
Additional Information
- Provide skilled nursing care to home health and hospice patients including wound care, IV therapy, and chronic disease management
- Support and supervision available after hours through an administrator, RN coordinator, or office support supervisor
- Scheduling is coordinated by the office
- Dress code requires solid color, clean, and professional scrubs
- All clinicians must clock in and out at the facility
- Candidates who previously worked at the facility must have an end date at least 6 months prior to consideration
- Candidates living within 80 miles of the facility are considered local for pay purposes
