Description
American Traveler is seeking a Physical Therapist for a Home Health role requiring home health experience and an active Michigan PT license.
Details
- Home health setting covering Van Buren, Calhoun, and Kalamazoo counties
- Full-time day shift schedule, 5x8-hour shifts from 8:00 AM to 5:00 PM
- Weekend rotation required every 4-6 weeks
- No call requirement
- 13-week contract assignment
Requirements
- Active Michigan PT license required (license required if applicable)
- Current BLS certification (AHA) required
- Minimum 1 year of home health experience required
- Valid, active driver's license in good standing required
- Daily access to a personal vehicle for business use and proof of auto insurance required
- Ability to evaluate, plan, and administer PT treatment for patients with injury or musculoskeletal, neurological, and orthopedic conditions
Additional Information
- Responsibilities include evaluating patient function, establishing treatment plans, and documenting care in the electronic health record per compliance timelines
- May design and fabricate splints or other assistive devices as needed
- Provides family and community education and participates in interdisciplinary team discussions
- May cross-train and provide coverage at other departments within Rehabilitation Services as needed
- Serves as a resource and provides in-services in areas of clinical expertise
- Proof of flu vaccination required with consideration; exemption requests must be submitted within 48 hours of signing
- References must be from a supervisor-level contact
- RTO limited to no more than 10 days
- Local candidates accepted at the same rate with no radius restrictions
- Previous internal travelers may return after 6 months; former permanent staff may return after 1 year
