Description

American Traveler is seeking a Physical Therapist for a Home Health position requiring at least one year of acute care PT experience and a valid OH PT license.

Details

Home health setting providing physical therapy services to patients in Montgomery County, Ohio
Full-time schedule including Saturday and Sunday plus one weekday, day shifts (8:00 AM-5:00 PM)
13-week assignment length

Requirements

Minimum of one year of experience as a PT in an acute care setting required
Home care experience desired
BLS certification required
OH PT license required if applicable
Valid driver's license and auto insurance with candidate listed as a driver required
Experience with HomeCare HomeBase charting system preferred but not required
Strong communication skills with patients, families, physicians, and co-workers
Knowledge of therapy best practices and sound clinical judgment
Detail-oriented with strong time management and prioritization skills

Additional Information

Responsibilities include patient assessment, evaluation, discharge planning, and development of short and long term care goals
Utilizes OASIS documentation and develops individualized plans of care
Performs assistive device and wheelchair assessments, including crutches, gait belts, and walkers
Provides home exercise programs, patient and family education, and safety/injury prevention guidance
Knowledge of Medicaid, Medicare A, and Medicare B billing preferred
First-time travelers not considered for this role
