Description

American Traveler is seeking a Physical Therapist for a Home Health position requiring EPIC experience and a valid driver's license.

Details

• Home Health setting providing personalized in-home care to patients throughout the community
• Day shift schedule, 8:00 AM-4:30 PM
• Requires EPIC experience
• Mileage reimbursement provided in addition to pay rate

Requirements

• Doctor of Physical Therapy (DPT) degree required if licensed on or after January 2016, or foreign equivalent
• Physical Therapy license required in all applicable states of practice (license required if applicable)
• Current BLS certification required
• Valid driver's license required
• Must be at least 21 years of age or older
• Minimum 2 years of experience in specialty required, with recent experience within the last 3 months
• EPIC experience must be noted in employment history

Additional Information

• Plans and conducts treatment programs designed to restore and improve physical, social, and mental function
• Serves as co-leader in an interdisciplinary approach to patient care
• Completes evaluations and establishes individualized treatment and discharge plans
• Provides therapy interventions using appropriate protocols and techniques
• Completes comprehensive and accurate clinical documentation within established deadlines
• Supervises assigned assistants
• Required vaccinations are a condition of employment, with medical and religious exemptions where applicable
• Accepts local candidates at the same rate as travel positions with no radius rule
• Candidates with more than 5 days of requested time off will be automatically declined
• Candidates with a prior adverse action on their license will not be considered
