Description

American Traveler is seeking a Home Health RN Care Manager requiring a California RN license and EPIC experience for a day shift skilled nursing role.

Details

• Home Health - Skilled Nursing unit providing personalized in-home care
• Manages a patient caseload, coordinating care across disciplines
• Day shift schedule, 8:00 AM to 4:30 PM
• EPIC experience required

Requirements

• Active California RN license required
• BLS/CPR certification from an approved vendor required
• Valid driver's license required, candidate must be at least 21 years old
• Current automobile insurance and a reliable personal vehicle required
• BSN preferred
• Acute care facility or home care experience preferred
• Candidates with a prior adverse licensure action will not be considered

Additional Information

• Performs initial and ongoing comprehensive patient assessments and updates plans of care as needed
• Educates patients and family members on healthcare and nursing needs
• Documents patient care, status, and response to treatment in accordance with policy
• Collaborates with other care team disciplines to meet patient care goals
• Complies with HIPAA and all applicable documentation standards
• No more than 5 days of requested time off (RTO) will be considered
• Local candidates accepted with no radius restriction; local pay rates match travel rates
