Description

American Traveler is seeking a Physical Therapist for a Home Health assignment, requiring 1+ years of home health experience and a valid driver's license for patient visits.

Details

• Home Health setting providing skilled visits for Medicare, Medicare Advantage, and Commercial insurance patients
• Patient population includes med/surg, cardiac, ortho, post-surgical, COPD, CHF, pneumonia, CVA, falls, weakness, Parkinson's, failure to thrive, neuro, trachs, trauma, and amputee patients
• Caseload of 6-8 home visits per day (minimum of 6 points daily, may increase at manager's discretion)
• Day shift schedule, 5x8 hours (8:00 AM-5:00 PM)
• 1-2 weekends required per contract
• Uses BayCare Forms, BayCare Scheduling, and Ace Timesheet documentation systems
• Laptop, MiFi, and portable printer provided for documentation and connectivity

Requirements

• Active state PT license required if applicable
• BLS certification required
• Minimum 1 year of experience required
• Home health experience required
• OASIS training preferred
• Cerner experience preferred
• Valid driver's license and personal vehicle required for home visits
• Proficiency with equipment including gait belts, walkers, sliding boards, collars, wheelchairs, and supplemental oxygen devices
• Experience with ortho, neuro, and cardiopulmonary patient populations
• Documentation skills including assessments, evaluations, plans of care, and OASIS

Additional Information

• Responsibilities include patient assessment, home exercise programs, family education, and safety/injury prevention instruction
• Must be able to identify and report signs of physical, sexual, or psychological abuse
• Billing knowledge across Commercial, Medicare A, Private Pay, and Workers' Compensation required
• Must be COVID vaccinated or provide exemption documentation
• Flu shot or declination required, with mandatory masking if declined
• Any solid color scrub accepted for dress code
• Local candidates accepted at the same pay rate
