Description
American Traveler is hiring an RN Case Manager with an active OH or compact license and 3+ years of acute care case management experience for a day-shift position with no weekends, holidays, or call.
Details
- Acute Medical and Surgical patient population
- Typical caseload of 20 patients per day
- Days shift, Monday through Friday, 7:30am-4pm
- No call, no holidays, and no weekends required
- 13-week assignment
Requirements
- Active RN license (Ohio or Compact)
- BLS certification required
- Minimum 3 years of Acute Care Case Management experience required
Additional Information
- Responsible for assessing discharge needs, social determinants of health, and discharge planning/referrals
- Must reside outside a 60-mile radius of the facility
