Requirements

• Active Physical Therapist license required (license required if applicable)
• Compact license accepted to start, with full state license required shortly after starting the assignment
• Experience in the outpatient setting required
• Current BLS certification may be required
• 1 professional reference required for consideration
• Social Security Number required for consideration
