Description

American Traveler is seeking a Home Health RN with HCHB and OASIS experience for a field position requiring in-home patient care and on-call rotation.

Details

• Home health setting providing in-home patient care to a geriatric population
• Field RN role involving interdisciplinary care coordination and plan of care development/oversight
• Averages 4-6 patients per day, with 24-30 points per week required
• Charting completed at the patient's home using HCHB and OASIS systems
• Day shift schedule, Monday-Friday 8:00 AM-5:00 PM
• Weekends as needed and participation in on-call rotation required, including at least one weeknight of call
• Must be willing to travel up to 50 miles outside the home base office
• Mileage reimbursed at IRS standard rate

Requirements

• Active TX or compact RN license in hand at time of consideration, pending licenses not accepted
• Current BLS/CPR certification required
• Minimum 1 year of Home Health nursing experience and 2 years overall clinical nursing experience required
• HCHB and OASIS experience required
• Current driver's license required
• Must have reliable transportation and insurance
• No history of license actions accepted

• Administer skilled nursing care to patients in diverse home environments
• Coordinate care with patients, families, certifying providers, and the interdisciplinary team
• Perform comprehensive patient assessments and collaborate on Plan of Care development
• Non-smokers preferred due to sensitive patient population with COPD
• Scrubs required, any color
• First-time travelers are welcome to apply
• Locals accepted within a 50-mile radius of the home base office
• CHPN certification is preferred but not required
