Description
American Traveler is seeking a Physical Therapist for a Home Health float position requiring MA licensure and 1 year of certified home care experience.
Details
- Home health setting providing physical therapy services to patients in their place of residence
- Float role covering the North Region including Andover, Bedford, Billerica, Burlington, Concord, Hanscom, Lawrence, Lexington, Lincoln, Nutting Lake, Pinehurst, Reading, Stoneham, Tewksbury, Wakefield, Wilmington, Winchester, and Woburn
- Patient population ranges from birth to over 100 years of age with physical impairments, functional limitations, or disabilities
- Shift schedule 08:00-16:30
- Weekend rotation once every 6 weeks
- Must be able to travel to patient homes as assigned
- Uses Epic for charting
Requirements
- Active Massachusetts PT license required if applicable; no conditional or pending licenses accepted
- Minimum 1 year of certified home care experience within a certified home health agency required
- Minimum 1 year of case management experience required
- Hospice, private care, or standalone case management experience does not meet the home health care requirement
- Flu vaccine required with no religious or medical exemptions allowed
Additional Information
- Collaborate with patients, families, physicians, and allied health professionals to identify needs and coordinate care
- Maintain thorough, timely electronic documentation in accordance with agency policies and standards of practice
- Start date contingent on credentialing clearance; delays may result in a rescheduled start date
- Local candidates within 60 miles of the primary facility may be considered at a reduced rate
- No validated parking available at Boston locations; public transportation or paid parking required
