Description

American Traveler is seeking a Home Health Physical Therapist with 2+ years of experience and home health background to provide skilled visits for a diverse patient population.

Details

Home health setting providing skilled visits for cardiopulmonary, geriatric, neuro, ortho, and oncology patients
6-8 home skilled visits per day (minimum of 6 points, may increase by 1 at manager's discretion)
Day shift schedule, 5x8-hour shifts from 8:00 AM to 5:00 PM
Every 3rd weekend required
Self-scheduled visits and hours based on office needs
Uses BayCare Forms, BayCare Scheduling, and Ace Timesheet documentation system
Utilizes laptop, MiFi, and portable printer for fieldwork

Requirements

Current BLS certification required
Active Florida or Compact PT license required (if applicable)
Minimum 2 years of experience required
Home health experience required; OASIS training preferred
Valid driver's license and personal vehicle required for home visits
Proficient in patient assessment, cardiopulmonary/circulatory care, and documentation
Experience with ortho and neuro patient populations
Skilled in use of home health equipment including crutches, gait belt, hospital bed, walkers, slings, sliding boards, collars, and supplemental oxygen delivery
Knowledge of home exercise program development and patient/family education
Ability to recognize signs of physical, sexual, and psychological abuse
Cerner charting experience preferred

Additional Information

Responsibilities include home exercise program implementation, medication education, and safety/injury prevention teaching
Must be COVID vaccinated or have an exemption on file
Flu shot or declination required; mask must be worn at all times if declined
Any solid color scrub accepted
Accepts local candidates at the same rate
Former BayCare employees must wait at least 12 months before returning as a traveler
RTO is not permitted in travel contracts for this facility
