Description
American Traveler is seeking a Physical Therapist for a Home Health and SNF assignment requiring at least 1 year of experience.
Details
- Split setting across SNF and Home Health, approximately 50/50 with potential fluctuation
- Occasional coverage may be needed at Hastings or Grand Island locations for SNF or HH
- Supervises 2 PTAs
- Uses Net Health for SNF documentation and Homecare Homebase (HCHB) for Home Health documentation
- Schedule is 5 days a week, approximately 8 hours per day, days shift
- Productivity expectations include 80% for SNF and approximately 6 patients per day for HH (28 patients per week at 90% productivity if HH only)
Requirements
- Minimum 1 year of experience required
- BLS certification required
- License required if applicable
- New graduates accepted
- Valid driver's license required for consideration
- Two professional references required: one supervisor from within the last 12 months worked and one peer or supervisor from within the last 3 years worked, including dates of employment and eligibility for rehire
Additional Information
- Scheduler and contact person available onsite; Therapy Supervisor is on maternity leave until mid-November with the Director serving as point of contact during that time
- No on-call or call-back requirements
- Local candidates accepted at a lower rate
- 80-mile radius rule applies for travel rate eligibility
