Description
American Traveler is seeking a Hospice RN for a home health assignment requiring 2+ years of experience and a ND or compact RN license.
Details
- Home health setting providing hospice/palliative care to assigned clients
- Primary shift schedule of 0800-1630, Days
- Requires 1-2 overnight on-call shifts per week (1630-0800) with rotating weekends and holidays
- Uses other computerized charting systems not otherwise listed
- Must have MN license or be able to obtain a temporary license to support cases in MN
Requirements
- Active ND or compact RN license required
- BLS certification through AHA required
- Minimum 2 years of nursing experience required
- Graduate of an accredited school of nursing
- BSN not required for this position
- Must be at least 18 years of age
Additional Information
- Provides hospice and palliative care to clients and families in a safe, coordinated, and compassionate manner
- Performs initial and ongoing client assessments and participates in developing individualized plans of care
- Coordinates with interdisciplinary team members and processes physician orders
- Completes required documentation, quality assurance audits, and educational in-services within appropriate timeframes
- Explains advance directives and available interdisciplinary services to clients and families
- Works independently with minimal supervision, seeking guidance as needed
- Local candidates within 80 miles of the facility may be considered at an adjusted rate
- Returning employees with prior Sanford Health employment must disclose this for review
