Description

American Traveler is seeking a Physical Therapist for a Home Health position requiring 1+ years of home health experience and knowledge of HCHB and OASIS.

Home Health setting
Day shift, 5x8s, Monday through Friday, no weekends
No call required
Mileage reimbursed per government rate

Requirements

Doctor of Physical Therapy (DPT) degree required if licensed on or after January 2016, or foreign equivalent
Active PT license required in state of practice (license required if applicable)
Current BLS/CPR certification required
Valid driver's license required; must be at least 21 years of age
Minimum 1 year of home health experience required
EPIC experience required
Knowledge of HCHB and OASIS required

Additional Information

Plans and conducts treatment programs to restore and improve physical, social, and mental functions
Serves as co-leader in an interdisciplinary approach to patient care
Completes evaluations, establishes individualized treatment and discharge plans, and communicates plans to appropriate parties
Completes comprehensive and accurate clinical documentation within established deadlines
Supervises assigned assistants
Required vaccinations are a condition of employment, with medical and religious exemptions available
Accepts local candidates with no radius rule; local rates match travel rates
Candidates with more than 5 days of requested time off will not be considered
