Description

American Traveler is seeking a Physical Therapist for a home health position requiring at least 1 year of home health PT experience and a valid driver's license.

Details

Home health setting covering south Lake County
Ortho case load with 4-6 visits per day
Schedule is Monday through Friday, 8:30am-5pm
1 weekend rotation per month required for ortho case visits
Coverage required for 1 major and 1 minor holiday
No on-call requirement

Requirements

Minimum 1 year of home health PT experience required
Valid driver's license and auto insurance required
License required if applicable
SSN and DOB required for consideration
1 professional reference from entire work history required
COVID vaccination card or approved declination required

Additional Information

Weekend or holiday call-offs may result in rescheduling to another weekend or holiday per facility policy
Mileage varies by week based on caseload
Local candidates accepted at the same rate as travelers
No radius rule in place
Former permanent employees must have a 90-day break in service before returning as a traveler
