Requirements

• Active PT license required; license required if applicable
• Experience in the outpatient setting required
• Can start with compact license but must obtain full state license while on assignment
• 1 professional reference required
• SSN required for consideration

• Candidates previously employed at this facility on a perm or PRN basis must have been separated for at least one year to be eligible for rehire
