Description

American Traveler is hiring a Registered Nurse for a Home Hospice and Palliative Care position requiring two years of home hospice RN experience and a NJ or compact RN license.

Details

• Home hospice and palliative care setting providing in-home patient visits
• 8a-4p shift schedule with every other weekend and comp days during the week
• Floating to different counties as needed based on demand
• No on-call responsibilities
• Uses EPIC for charting

Requirements

• Active NJ or compact RN license required
• Valid New Jersey driver's license required
• Current BLS certification required
• Minimum two years of RN experience in a home hospice environment required
• Associate's (ASN) degree or diploma with NJ RN license required
• EMR charting experience required
• Strong critical thinking and communication skills required
• Knowledge of hospice regulations, pharmacology, and safe medication management required

Additional Information

• Performs patient assessments to determine hospice appropriateness, learning needs, and progress toward care goals
• Develops, implements, and revises individualized plans of care for patients and families
• Completes admission, discharge, and re-visit documentation within required timeframes
• Provides age-appropriate patient and family education and evaluates teaching effectiveness
• Manages caseload priorities and coordinates with the interdisciplinary team (IDT)
• Attends and participates in IDT meetings every two weeks and monthly staff meetings
• Supervises home health aides per agency standards
• Maintains a safe environment through infection control practices and medication reconciliation
• Participates in performance improvement activities and may mentor new employees
• May float to a nearby sister hospital as needed
