Description

American Traveler is seeking a Home Health Physical Therapist with 1+ years of experience treating geriatric patients with strokes and orthopedic conditions.

Details

• Home Health setting serving adult to geriatric patients
• Focus on stroke, orthopedic, and geriatric caseloads
• Typical caseload of 7 visits per day with productivity around 25 units for full time
• Day shift schedule, 5x8 hours, 8:00 AM to 4:30 PM
• No weekend shifts required
• May float within facility and scope of practice as needed
• Uses Homecare Homebase (HCHB) charting system, transitioning to Epic around September 2026
• Mileage reimbursed at IRS standard rate

Requirements

• Minimum 1 year of experience required
• BLS certification required
• GA PT license or Compact PT license required; license required if applicable
• Experience with home health, geriatrics, strokes, and orthopedic conditions required
• Skilled in patient assessment, adaptive and assistive devices, and body mechanics
• Experience with documentation including OASIS, plan of care, evaluations, and reassessments
• Proficient with equipment such as gait belts, Hoyer lifts, walkers, and sliding boards
• Experience treating hip fractures, total hip and knee replacements, and lower extremity amputations preferred
• Experience with dementia, Alzheimer's, and Parkinson's disease patients preferred
• Strong skills in home exercise programs, medication management, and patient/family education

Additional Information

• Responsibilities include evaluations, treatment planning, home exercise programs, and patient/family education
• If no HCHB experience, an online course must be completed prior to starting
• Scrubs of any color are accepted
• Maximum RTO of 7 days per contract, must be finalized at time of submission
• No RTO requests permitted during orientation week
• Holiday coverage required based on number of major holidays falling within the assignment
• First-time travelers are accepted for this role
• Local candidates accepted at the same rate
• Prior Piedmont employees must be away from all Piedmont facilities for one full year before returning as a traveler
• Returning travelers who were cancelled or not approved for return must complete one assignment outside Piedmont before reapplying
