Description

American Traveler is seeking a Home Health RN with 2 years of home health and acute care experience for a Monday-Friday daytime schedule with no weekends or on-call.

Details

• Home Health unit setting for adult patients
• Primary diagnoses include CHF, hypertension, diabetes, total joint replacement (TJR), and oncology/cancer cases
• Day shift schedule, 5x8 hours, 8:00 AM to 4:30 PM, Monday through Friday
• No weekend or on-call requirements
• Matrix EMR strongly preferred
• Uses technology devices including laptop, iPad, and iPhone for documentation

Requirements

• Active RN license (PA or Compact) required
• BLS certification required
• OASIS certification preferred
• Minimum 2 years of home health experience required
• Acute care experience preferred
• Above-average technical knowledge required for use of documentation devices
• Experience with start of care, admission assessment, case management, recertification, and related home health documentation preferred
• Skills in wound care, medication management, catheter management, and IV/central line care preferred
• One supervisory reference from within the past year required for consideration
• Completion of lab work within 72 hours of signing contract required

Additional Information

• Provide comprehensive home health nursing care including assessments, medication management, wound care, and disease management for chronic conditions
• Coordinate care services and document using OASIS and standard home health documentation guidelines
• Candidates must complete PA fingerprinting and ChildLine registration within 48 hours of signing a contract
• PA fingerprinting must be completed within Pennsylvania in most cases
• Candidates must be responsive and cooperative throughout the credentialing process to avoid contract risk
• First-time travelers will be considered if they meet the experience requirements
• Travel pairs will be considered for this position
• Must reside outside a 50-mile radius of the facility to be eligible for travel rates
• Previous permanent or per diem employees must have a break in service of at least 18 months before returning as a traveler
