Description
American Traveler is seeking a Home Health Physical Therapist with 2+ years of experience and current Montana PT licensure to provide skilled therapy services to patients in their homes.
Details
- Home Health and Hospice setting
- 36 hours per week, day shift
- 13-week contract
- Travel to patient homes and other care settings to provide therapy services
Requirements
- Current PT license in the state of Montana required, if applicable
- Minimum 2 years of experience required
- BLS certification required
- Valid driver's license required
Additional Information
- Receive and review physician therapy orders for Home Health and Hospice patients
- Complete comprehensive patient evaluations and establish written treatment plans with short-term and long-term goals
- Provide skilled interventions including therapeutic exercises, stretching, mobility training, balance activities, pain management techniques, and functional rehabilitation
- Assess home safety and recommend equipment, assistive devices, or environmental modifications to promote independence and prevent falls
- Monitor patient progress and modify treatment plans as needed to achieve optimal outcomes
- Educate patients, caregivers, and family members on treatment plans, home exercise programs, safety measures, and expected outcomes
- Maintain accurate and timely documentation including evaluations, progress notes, discharge summaries, recertifications, and resumptions of care
- Collaborate and communicate with physicians, nurses, therapists, and other healthcare professionals involved in patient care
- No holiday coverage required
