Description

American Traveler is seeking a Physical Therapist with 2+ years of experience for an outpatient, long term care, home health, and inpatient rehabilitation role.

Details

Work spans outpatient, long term care, home health, and inpatient settings
Treats patients of all ages with developmental deficits, physical injury, or illness
Uses EPIC for documentation
8-hour shifts scheduled Tuesday through Saturday
Holiday coverage required

Requirements

Minimum 2 years of experience required
Current, active state PT license required
License required if applicable
Experience across outpatient, long term care, home health, and inpatient settings preferred

Additional Information

Coordinates PT services and provides direct therapeutic care to assigned clients
Teaches rehabilitation skills to clients and caregivers to promote independent functionality and safety
Develops and revises individualized plans of care based on assessments, diagnoses, and physician orders
Applies treatments using physical agents such as heat, water, and electricity with equipment like whirlpool baths, electrical stimulation, and ultrasound
Documents all care thoroughly and in compliance with Medicare, state, federal, JCAHO, and payer requirements
May supervise Physical Therapist Assistants and Home Health Aides as applicable
Participates in staff meetings, case conferences, and quality improvement programs
