Description

American Traveler is seeking a Home Health Physical Therapist with 1+ year of recent experience and proficiency in HCHB documentation to serve a geriatric patient population.

Details

• Home health setting serving a geriatric patient population
• Day shift, 5x8-hour schedule, 8:00 AM - 4:30 PM
• Average of 5-7 patients per day, minimum 30 points weekly
• On-call rotation may be required
• Territory covers up to a 60-mile radius from the hub office, with flexibility required across the territory
• All charting must be completed by 9:00 AM the following morning

Requirements

• Minimum 1 year of recent home health physical therapy experience required
• BLS certification required
• OASIS experience required for staff completing Start of Care
• Proficiency in HCHB (Homecare Homebase) EMR required
• Active driver's license and personal transportation required
• License required if applicable, and must be in-hand at time of submission

Additional Information

• Main responsibilities include home exercise programs and patient/family education
• Timely communication and documentation are mandatory
• Monday orientation includes billable modules
• Hospice/Home Health checklist is mandatory for consideration
• Scrubs required in any color; jeans not permitted
• Cell phone not provided; iPad provided for documentation
• Mileage reimbursed at the IRS standard rate
• First-time travelers will not be considered for this role
• Former permanent employees must be separated for at least 6 months and approved by HR before returning as a traveler
• Travel pairs accepted, but each traveler will be assigned their own territory and must have their own transportation
• Local candidates accepted at the travel rate
• Requested time off must be submitted at the time of application
