Description

American Traveler is seeking a Physical Therapist for a Home Health position requiring 1+ year of recent experience and proficiency in HCHB and OASIS.

Details

Home Health setting serving a geriatric patient population
Territory covers up to a 60-mile radius from the hub office, with flexibility required across the territory
Day shift schedule, 5x8 hours (8:00 AM - 5:00 PM)
Average caseload of 5-7 patients per day
On-call rotation required approximately every 3rd to 4th weekend
Uses HCHB EMR system

Requirements

Minimum of 1 year of recent, relevant Home Health PT experience required
BLS certification required
OASIS experience required for completing Start of Care (SOC) documentation
Proficiency in HCHB (Homecare Homebase) required
Active driver's license and personal transportation required
License required if applicable
Strong documentation and communication skills required

Additional Information

Main responsibilities include home exercise program development and patient/family education
Treats orthopedic, neurologic, and post-surgical conditions including post-stroke rehabilitation and spinal cord injury
Scrubs required in any color; jeans not permitted
iPad provided for documentation; personal cell phone use not required for work purposes
Mileage reimbursed at the IRS standard rate
Monday orientation required, including completion of billable modules
Travel pairs accepted, though each traveler will be assigned to their own territory and must have independent transportation
Timely documentation and communication are mandatory expectations of the role
RTO requests must be submitted at the time of application
Former permanent employees must be separated for at least 6 months and approved by HR before returning as a traveler
First-time travelers will not be considered for this position
Local candidates accepted at the travel rate
