Description

American Traveler is seeking a Physical Therapist for a Home Health assignment in Southern Oregon, requiring 1+ year of skilled home health experience and OASIS proficiency.

Details

Home Health setting covering Medford, OR and surrounding areas
Day shift, 8-hour shifts, 5 days per week (4x10s and 3x12s not accommodated)
1:1 patient ratio, visiting up to 6 patients per day with a 30 productivity point standard
No weekend or on-call requirements
No floating between units required
Charting performed in Home Care Home Base/OASIS

Requirements

Minimum 1 year of skilled home health experience required
BLS certification required
OASIS experience required
License required if applicable
Must have reliable personal transportation for home visits
Primary series COVID vaccination plus one booster required

Additional Information

Scheduling system groups clients to minimize drive time, with occasional travel radius of 50+ miles
Any color scrubs accepted
Free parking available
Mileage reimbursed at IRS standard rate
First-time travelers will not be considered for this role
Local candidates accepted within a 50-75 mile radius at the same pay rate
Facility may cancel up to one shift per two-week pay period
