Description

American Traveler is seeking a Home Health Physical Therapist with at least 1 year of relevant experience and proficiency in HCHB documentation.

Details

Home health setting within a PT Home Health unit
Day shift schedule, 5x8 hours from 8:00 AM to 4:30 PM
On-call rotation may be required; details to be discussed during interview
Travel pairs accepted, though each traveler is assigned to their own territory and must have personal transportation

Requirements

Minimum of 1 year of relevant home health physical therapy experience
OASIS experience required for staff completing Start of Care (SOC) documentation
Proficiency in HCHB (Homecare Homebase) charting system required
Active driver's license and personal transportation required
License required if applicable
Prior travel experience required; first-time travelers will not be considered

Additional Information

Timely communication and documentation practices are mandatory
Orientation begins on Mondays and includes completion of billable training modules
Hospice/Home Health checklist completion is mandatory
Requested time off (RTO) must be submitted at the time of application
Former permanent employees must be separated for a minimum of 6 months and approved by HR before returning as a traveler
