Description
American Traveler is seeking an experienced RN Case Manager with an active AK license for an outpatient ambulatory care position requiring recent case management experience.
Details
- Outpatient ambulatory clinic setting providing primary care case management services
- Patient population includes individuals and families across the continuum of care
- Monday through Friday schedule, 8:00 AM to 5:00 PM
- 13-week assignment with 40 guaranteed hours per week
- Floating to other departments or assignments as needed
- Documentation completed in the facility's electronic health record
Requirements
- Active, unrestricted Alaska RN license required at time of submittal
- BLS certification required
- Case Management certification preferred
- Minimum of 1 year of case management experience in a clinic or hospital setting within the last 2 years required
- Graduate of an accredited school of nursing
- Bachelor's degree preferred
- High school diploma or equivalent required
Additional Information
- Performs comprehensive patient assessments and develops individualized plans of care
- Administers medications and treatments per provider orders and policy
- Provides patient and family education related to care, medications, and discharge needs
- Coordinates care with providers and interdisciplinary team members, including referrals and follow-up services
- Advocates for patients and assists them in navigating the healthcare system
- Delivers culturally responsive, patient-centered care
- Exercises independent nursing judgment consistent with Alaska Board of Nursing scope of practice
- Participates in quality and performance improvement activities and reports adverse events per policy
- Provides guidance and delegation to ancillary staff and supports orientation and mentoring of staff and peers
- Start date will be no less than 6 weeks after fingerprint cards are received by the facility
- Housing and transportation are not provided by the facility
