Description

American Traveler is seeking a Home Health Physical Therapist with 2+ years of experience and EPIC proficiency for a 13-week day shift assignment.

Details

• Home Health and Hospice Care setting
• Day shift, 5x8 hours, 8:00 AM - 4:30 PM
• No weekend requirement
• Must be willing to float based on unit needs
• Uses EPIC for documentation

Requirements

• NH license required or compact PT license (license required if applicable)
• Minimum 2 years of experience required
• AHA or American Red Cross certifications only accepted
• References required, with one pending reference accepted to begin the process
• Drug screening must be completed within 14 days of start date
• Required skills include lifting and transfer, patient transport to/from treatment, scheduling appointments, and telephone calls
• Wound care certification preferred
• Experience in cardiopulmonary, geriatrics, neurology, oncology, orthopedics, pediatrics, sports medicine, and women's health preferred
• Experience with OASIS and RUG levels preferred
• Experience supporting ER, ICU, and Med/Surg units preferred

Additional Information

• Responsibilities include home exercise programs, patient and family education, medication management, and safety/injury prevention teaching
• Documentation duties include assessments, evaluations, discharge planning, long and short term goals, and reassessments
• Billing experience with commercial insurance, Medicaid, Medicare A/B, private pay, and workers' compensation preferred
• Open to first-time travelers
• Former employees of the facility are not eligible for consideration
• Local candidates residing within 50 miles of the facility are accepted at a reduced local rate
• All requested time off must be submitted upfront
