Description
American Traveler is hiring a Home Health RN with at least 1 year of home health nursing experience and required proficiency in Homecare Homebase EMR.
Details
- Home health nursing position making visits throughout University City, Cornelius, Davidson, Kannapolis, Salisbury, Concord, Huntersville, and surrounding areas
- Average of 60-80 miles driven daily with mileage reimbursed weekly
- Required to complete 6 patient visits per day including routine visits, discharges, ROC, recerts, and reassessments
- Patient population includes chronic disease management, wound care/wound vac, IV therapy, and pleurx management
- Case management responsibilities overseeing 25-30 patients at once
- 8-hour day shift schedule, 8:30am-5pm, 40 hours per week
- Weekend rotation not required unless interested
- Call not required but available for interested candidates
- Schedule cycle is every 13 weeks
Requirements
- Active RN license (NC state or Compact)
- BLS certification required
- Valid Driver's License and proof of insurance required
- Minimum 1 year of Home Health Nursing experience required
- Homecare Homebase EMR charting experience required with no exceptions
- Skills required include IV therapy, pleurx, wound vac, wound care, patient assessment, disease teaching, and OASIS
- Last 4 digits of SSN and full date of birth required for consideration
- Minimum 1 professional reference dated within the past year from a Supervisor, Manager, Director, or Charge RN
Additional Information
- Schedule patient visits the day prior to the planned appointment
- Utilize EPIC Rover (view only) and Focura to upload discharge instructions and documentation such as wound photos
- Samsung Note 9 phone provided for documentation and patient/provider communication
- Support available from departmental leadership, scheduler, Clinical Manager, Clinical Supervisor, and clinical peer team
- Orientation includes New Employee Orientation (NEO) and 5 days with a preceptor
- Holiday coverage expected for all holidays unless manager approval is received for time off
- RTO requires manager approval
- Navy blue scrubs required
- Free parking available
- Team-oriented department culture
- Must reside at least 50 miles outside the home unit to be eligible for this position
- First-time travelers will be considered if experienced in home health nursing
- Local candidates not accepted due to radius rule
- Returning employees must be separated from any Advocate Health Southeast facility for at least 1 year after working a permanent position
